In order to make your visit to our practice as efficient and as personalized as possible, we ask that you complete the registration paperwork prior to your appointment. If you have completed your paperwork prior to your appointment, please arrive at least 10 minutes before your appointment time. If you have not completed your paperwork prior to your appointment, please arrive 15 minutes before your scheduled appointment time.

If you are running late, please contact us at (510) 444-3297. Please be advised that if you are late 15 minutes or more we may need to reschedule you appointment.

Please be sure to bring the following items with you:

• Current insurance card
• Referral form from your doctor's office
• Your completed registration forms
